WHAT YOU NEED TO KNOW
Fill out the "Order Self Screening Kits" order form.
Once order is received and packaged you will be notified by your local area Chamber of Commerce. Please do not visit the designated pickup location until your time for pickup has been scheduled.
Each company must assign a Screening Supervisor Designate, who will be the contact person.
If you have multiple businesses, please fill out one form per business.
If you cannot arrive promptly at your selected time, your tests will no longer be available and you will need to reschedule.
BEFORE YOU ARRIVE
All visitors must complete and PASS a COVID-19 assessment before entry into the premises. (Available HERE.)
Check in at registration and provide the completed PASP (Provincial Antigen Screening Program) form. (Available HERE.)
You will be required to watch an 8-minute training video during your initial pickup. (Available HERE.)
Capacity limited to 1 visitor at any one time at each designated pickup location.
Masks and physical distancing are mandatory while at the pickup site.
REPORTING YOUR RESULTS
In order to participate in the Workplace Self Screening program, all participants must agree to immediately report the results as soon as employees have been tested. Your Screening Supervisor Designate will be responsible for reporting all results. Results can only be submitted through the online portal. Results are shared with Health Canada and the Ministry of Health Ontario.
Anyone who receives a positive test result through the Workplace Self Screening Kit will need to make arrangements with their local Public Health Unit to receive a PCR test as soon as possible. They must also work with Public Health to determine when it is safe for them to return to work.

An ordering confirmation will be sent to the email provided with details of location and protocols. A Screening Supervisor Designate must be chosen to pick up the tests and will be required to watch an 8-minute instructional video on properly conducting the self screening, which can then be shared with all your employees. Do not send a relative, friend or driver for your pickup. You will receive 4 weeks worth of tests at the time of pickup.
If you are ordering refills, you must have reported your results as per the Provincial Antigen Screening Program Agreement. We cannot refill orders unless you have reported the results for 75% of the screening kits that you picked up.
Please do not visit the designated pick-up location until you have heard from us with confirmation of your appointment.
